About the Rural Arizona CoC

 
 

The Rural Arizona Continuum of Care covers all of the rural Arizona counties, excluding the urban counties of Maricopa and Pima.

Essentially, the current Rural AZ CofC is comprised of the thirteen of fifteen rural counties within AZ and the structure is a "bottoms-up" homeless planning process. The "Bottom" refers to the localized planning process held in each of these thirteen counties for which there are twelve HUD Homeless McKinney-Vento Act programs in and "Up" refers to the "Rural Statewide planning process in which a representative from each of the geographical areas is involved with on a quarterly (annual) basis.

Mission Statement 

Through a bottoms up (local community upwards to statewide level) planning process structure the mission of the Rural AZ CoC is to continually develop and provide essentially needed housing resources to individuals and families at risk or currently homeless throughout all thirteen (13) rural counties in Arizona.

Providers 

Following are the current HUD Project Sponsor agencies within the Rural AZ CoC that have implemented HMIS.  Agencies are listed in according to the numbers on the map shown above.

1. The Guidance Center/Inverrary House

Located within the city of Flagstaff, and Coconino County, this renewal SHP provides 8 units of permanent supportive housing (project based) for homeless individuals with permanent disability, serious mental illness or co-occurring disorder.

 

2. BOTHANDS/Sharon Manor

Located in the city of Flagstaff, within Coconino County, this renewal SHP provides 8 units of transitional supportive housing (project based) to individual women and and women with children who have experienced domestic violence.

 

3. Open Inn, Inc. - Prescott Homeless Youth

Located in the downtown Prescott area in Yavapai County, this renewal SHP provides 6 units of transitional supportive housing (project based) who have been referred by the emergency shelter or families due to subsequent homelessness.

 

4. United States Veterans Initiative

Located in the city of Prescott on site of the Veterans Affairs Hospital grounds, and within Yavapai County, this renewal SHP provides 58 beds of transitional housing and supportive services (employment re-engagement) for homeless veterans, some of whom have substance use disorders or serious mental illness.

 

5. United States Veterans Initiative

Located in Yavapai County, this new Permanent SHP provides 6 units of scattered site “Leasing” for individuals chronically homeless and of veterans’ status.

 

6. Colorado River Union High School District (CRUHSD) #2/Homeless Youth Project

Located in Bullhead City, County of Mohave, this renewal SHP/SSO provides supportive services in an "outreach model" program for 60 to 80 homeless you within the Bullhead City School District.

 

7. Mohave County Community and Economic Development

Located in Mohave County, this renewal project provides 13 units of permanent (scattered sites) housing for homeless persons with permanent disability, serious mental illness.

 

8. Old Concho Community Assistance Center (OCCAC)/Little Colorado PH

Located in Apache County, this renewal SHP provides permanent supportive housing for 6 homeless individuals with a permanent disability, serious mental illness or co-occurring disorder and 1 homeless family in one of 3 single family dwellings acquired through HUD funding from the originally approve continuum application and Grantee state funding.

 

9. Old Concho Community Assistance Center (OCCAC)/New Start TH Project

Located in Apache County, the renewal SHP provides 8 units of transitional housing (scattered sites) to homeless individuals and families.

 

10. Old Concho Community Assistance Center (OCCAC)/Navajo County TH

Located in Navajo County, this renewal SHP provides 10 units of transitional housing (scattered sites) for homeless individuals and families in an area of the state which purports some of the highest costs for housing.

 

11. Horizon Human Services PH

Located in the city of Casa Grande, within Pinal County, this renewal SHP provides permanent supportive housing for 8 homeless persons with a permanent disability, serious mental illness, co-occurring or substance use disorders at one of 4 newly purchased single family dwellings.

 

12. Community Action Human Resources Agency (CAHRA)/Dreamcatcher

Located in Pinal County, this renewal SHP provide 10 units of transitional supportive housing (scattered site model of both multi and single family dwellings) for homeless families.

 

13. Community Partnership of Southern Arizona (CPSA)/HOGAR Rental Assistance

Located in Cochise and Santa Cruz Counties, this renewal SHP provides 16 units of permanent supportive housing (scattered sites) for homeless persons with a permanent disability, serious mental illness, or co-occurring disorder.

 

14. Women's Transition Project

Located in the town of Bisbee, within Cochise County, this renewal SHP provides an employment training model program for up to 19 women at a time who reside in a non HUD funded transitional program.

 

15. The Excel Group PH

Located in Yuma County, this renewal SHP provides 20 units of rental assistance (scattered sites), affordable housing, and supportive services to homeless persons with a permanent disability, serious mental illness.

 

16. The Excel Group TH/Orange Avenue Apartments

Located in Yuma County, this renewal SHP provides 11 units of transitional supportive housing (project based) for homeless persons with a permanent disability, serious mental illness.

 

17. The Excel Group - SPC 25

Located in Yuma County, this renewal Shelter Plus Care provides 25 units of rental assistance with permanent affordable supportive housing for individuals and adults (with families) who have a permanent disability, serious mental illness, or co-occurring disorder.

 

18. CPSA Rural TRA/Shelter Plus Care  

This project is located in Graham, Greenlee (20 units), and Santa Cruz (5 units) Counties for a total of 25 units of permanent supportive housing in a scattered site model for "chronically homeless" persons/participants, per HUD definition, with a substance use disorder, serious mental illness, or co-occurring disorders. The SPC project has been funded for 5 years.

 

19. The EXCEL Group – SPC 25 La Paz and Yuma Counties    (new project)

This is a geographically combined new Shelter Plus Care project funded for 5 years totaling 25 units. 16 units are located in La Paz County and 9 are in Yuma County.  The project is serving persons chronically homeless and with a serious mental illness, co-occurring disorder, or substance use disorder.

In addition to the HUD Project Sponsors listed above, the Rural Arizona Continuum of Care includes many other providers of emergency shelters, transitional housing, permanent supportive housing, street/outreach, and supportive services in 13 of the 15 counties in Arizona.
